Surrogacy is a highly controversial and tricky legal process as it uses a surrogate mother to deliver a baby. Surrogacy is a process in which another female carries and deliver a baby for the couple needed child but not able to do so. There are four categories of surrogacy
Traditional: in this process, a female gets artificially inseminated with the male’s sperm and then carries, deliver the baby for the needed couple. A traditional surrogate is an infant biological mother as it was her egg that was fertilized by the male’s sperm. In this case, donor sperm can also be used. It is also known as partial or straight surrogacy. The techniques of IVF and IUI have been used in the process.
Gestational surrogacy: it is also known as full or host surrogacy. In this kind of surrogacy, eggs from the mother and sperm from the father are taken and fertilized in artificial condition then inserted it into the uterus of a gestational surrogate. A gestational surrogate does not have any kind of genetic links with the child as her egg was not used for fertilization. In this case, a gestational surrogate is known as the “birth mother” as biological mother is still the female whose egg is used for fertilization.
Commercial surrogacy: In this the surrogate mother is given ample compensation by the intended parents for carrying their child in her uterus.Commercial surrogacy is legalised in India.
Altruistic Surrogacy: no compensation or financial help is given for the surrogate woman who carries the child in her uterus for the intended parents.
Surrogacy is recommended only if a female has certain medical condition that makes it not possible or dangerous to get pregnant and give birth. These certain conditions include
Recurrent pregnancy loss
Malformation or absence of the womb
Failure of repeated IVF implantation
Surrogacy also makes possible parenthood to the couple who might not able to adopt a child. Although. It is a risky process. The risks associates with the surrogacy are
Ovarian hyper stimulation syndrome
The level of success in surrogacy is completely depends on certain factors such as maternal age, infertility causes, and male and female reproductive history.
In Vitro Fertilization is one of the assisted reproductive technologies, and is commonly referred to as IVF. IVF is the process of fertilization by separately extracting eggs and retrieving a sperm sample, and finally manually combining the egg and sperm in a laboratory dish. The embryo is then transferred to the uterus.
Polycystic ovarian syndrome (PCOS), is a condition in which the levels of the sex hormones present in women like estrogen and progesterone are not in balance. This condition leads to the unwanted growth of ovarian cysts. PCOS will cause problems with a women???s menstrual cycle, fertility, cardiac function and appearance.
Causes of PCOS :
Hormonal imbalances and genetics play an important role in causing PCOS. Women are more likely to develop PCOS if their mother or sister also has the condition. Overproduction of the hormone androgen may be another factor which causes PCOS. Androgen is a male sex hormone that women???s bodies also produce and when this hormone is produced in a level more than normal leads to PCOS. This can affect the development and release of eggs during ovulation. Also, excess insulin can cause high androgen levels.
What are the symptoms of PCOS ?
Excess hair growth on the face, chest, stomach, thumbs or toes
Thyroid function tests which are done to determine how much of the thyroid hormone the body produces.Fasting glucose tests to measure your blood sugar levels and insulin production.Lipid level tests to help assess the amount of cholesterol in the blood.
How is PCOS treated ?
A healthy diet and regular exercise is recommended for all women with PCOS. This helps to regulate the menstrual cycle and lower the blood glucose levels
Women who don???t want to become pregnant may be prescribed with birth control pills. These can help regulate the menstrual cycle and lower the levels of male hormones. If a woman with PCOS is suffering from infertility, then fertility drugs may be administered to aid ovulation
Diabetes medications are also prescribed to lower blood glucose and testosterone levels
Surgery may be recommended for some women with PCOS

What is IVF?
IVF is nothing but original ‘test-tube’ baby technique. It was developed more than 30 years ago. It was done for the treatment of women with damaged Fallopian tubes, and this remains an important reason for treatment today.
How does IVF work? IVF treatments begin with hormone therapy. It to stimulate the development of several follicles in the ovary. These are collected as eggs and then fertilized in a test-tube to create several embryos. After two and five days, one or two of these embryos are transferred through the vagina to the uterus. However, in IVF not every embryo implants to become a pregnancy, so that surplus embryos are frozen. Another transfer might be tried if the first one fails.

THe process is held under the supervision of an ultrasound, without the use of anesthesia. 3, Embryo culture – Monitoring the process of fertilization and embryo development. At the end of 4-6 hours after the follicle puncture is placed resulting in successful fertilization begins development of embryos by cell division. 4, Embryo transfer – The process of transporting the embryos into the uterus using a special catheter which is introduced through the cervical canal after approximately 72 hours after fertilization. Transfer the embryos are usually about 4 to greater likelihood of pregnancy. Embryo transfer process is absolutely painless and requires no anesthesia or analgesia.
How IVF is performed?
The next step described is how to do IVF. Under sterile conditions, the egg is placed in a vial with the medium was added to the sperm and kept in an incubator. After 3-5 days, the ready-made embryos transferred into the womb. Before that, genetic screening is conducted at times to identify fetal genetic abnormalities. Study of Couples is extremely important in adverse family history. Not all of these embryos are implanted. In Vitro Fertilization uses two or three embryos.
After IVF is performed, the women is prescribed with a maintenance therapy of human chorionic gonadotropin. Fourteen days is the recommended time period for performing a pregnancy test. Two strips can be used to say whether IVF Fertilization takes place or not. In most of the cases initial attempt is a failure. There is no need for being upset. There are many kids who are born through this born through this process. Every year this process is continued and help parents conceive.

Infertility is a common problem nowadays and the couple is said to be suffering from infertility when they are unable to conceive a child even after a regular contact of one year.
Now infertility is a complicated kind of problem which involves factors from the male partner as well as from the female partner, both of them are responsible to produce a healthy baby. So if there is any problem in any of the partners or sometimes we don’t even find the problem we can’t blame it on anybody which is called unexplained infertility.
Now the contribution to forming a baby, its one third problems could be because of the male factor, one third could be because of the female factor and about twenty percentage of the infertility cases are unexplained. Its the complicated process in the sense that there has to be the production of the healthy spermatozoa which can unite with healthy ovum or the egg and this egg, which is released from the ovaries goes through open tubes and gets implanted within the uterus. Now if there is problem in any of these factors then the pregnancy doesn’t happen.
Problems in males could be because of inadequate sperm count or unhealthy sperms which are seen very common because of life style problems, then there could be a decrease in the count or decrease in the mobility of the sperms which can also cause to male factor infertility.
While in females, there could be problem with factors which are responsible for infertility in females could be multi-port. Starting from the ovaries because the ovary is the main organ in the female body which produces the eggs. There could be a problem with the ovaries there could be problems with tubes. Tube is the organ which picking up eggs from the ovary, so if there is any problem the tubes may be unhealthy, they may be infected, they may be blocked. On any of the situation the healthy ovum, which is released from the ovaries is not picked up is not allowed to go to the uterus. There could be problem with uterine cavity, the uterine cavity could be having some kind of fibroids, some infections or some kind of sputum which will not allow the implantation of the embryo, so this could be the other important factor which could cause infertility in females.

Treatment of Infertility can be drugs or with surgical techniques, With drugs we can stimulate the ovaries to make healthy eggs, which then meet the sperm and the pregnancy can take place. Some times they could be structural abnormalities within the uterus or they could be some fibroids which can need to be surgically resected. Then also the need for surgery is there, some times there could be cornual block which will need to treat hysteroscopically or laparoscopically and then you can have some kind of hope that the lady is going to get pregnant.
When all other techniques of getting pregnant are unsuccessfull, then the patient is advised to go in for ivf. which is called In Vitro Fertilization
Ivf is a simple technique , in which the ladies eggs are formed by multiple hormonal injections .The eggs are then matured and they are surgically retrieved under anesthesia . These eggs are then taken in laboratory in the test tube, in which the sperms are put in and the embryos formed. When the embryos healthy at day 3 or day 5, the embryos are retransfered back in to the uterine cavity of the women. The result of ivf can be exist after 14 days of transfer of the embryos.
Ivf technique started in 1978 in UK and since then almost all countries now are very well experienced with this technique. They are getting good result with ivf.
